CONTRACT FOR EVERGREEN SUITES
This Contract is in
Addition
to the Terms and Conditions of the
Northern Paws Pet Lodge, LLC
Boarding Contract
1. I do hereby request my
pet(s) be located in the Evergreen Suites while staying at Northern Paws Pet
Lodge.
2. Owner agrees to pay for any damage to contents of the room, including:

| · | Sheet | (Price of new one at that time) |
| · | Blanket | (Price of new one at that time) |
| · | TV | (Price of new one at that time) |
| · | Miscellaneous damage to the room itself |

3. Northern Paws Pet Lodge
may remove bed, mattress, and blankets from suite if pet damages
them.
4. Your pet(s) may be
relocated from our Suite area to the East or West Pines for the following
reasons:

Excessive barking (disturbing other guests)

Excessive soiling of Suite (urination, etc)
-
Any signs of damage to Suite
-
We feel your pet is Emotionally not doing well in this private area.
(You will only be charged the “Suite Rate” for the night/s that your dog/s
stay in that room)

Owner has read and understands this Contract’s terms and
conditions, and is freely and voluntarily signing the contract knowing
that is valid anytime the listed pet(s) is/are boarded at Northern Paws
Pet Lodge, LLC. |
